Patients with Dental Insurance

Your first appointment with us is payable by you. In the future, after pretreatment authorization (PTA), all deductibles and co-payments must be paid at time of service. Note: If we are unable to confirm patient eligibility and complete insurance information, the full amount is payable by you.
All prosthetic restorations (known as major services and often include crowns, bridges, dentures and partial dentures) are recommended to be pre-authorized by your insurance carrier. Once preauthorization is completed and we have received an explanation of benefits (EOB), final payment may come from your carrier.
Not all insurance carriers will assign benefits to providers. Should your carrier not assign benefits to us, then final payment must come from you. We will do our best to obtain a profile of your dental insurance benefits and communicate with you how best to utilize your plan to obtain maximum benefits. We are here to help you receive the best quality of care.

Payment for treatment is due at time of service. We accept cash, checks, Visa and MasterCard.
For prosthetic services, 1/2 is required at start of treatment, and the balance over the remaining appointments. Full payment is expected prior to completion of the case. Extensive cases may require special arrangements. A payment schedule will be established for you at the treatment conference.
Finance charges will accrue at 1.5% per month, $2.50 minimum, on balances over 30 days old.

Emergency Care for New Patients

All emergency care patients are expected to pay 100% for their treatment at the end of their appointment. If you have dental insurance, we will submit this to your insurance company and we will have the payment sent to you.
